Safe Work Practices

1.  I will come to work mentally alert and physically able to perform all job duties.

2.  I will exercise extreme caution when it is absolutely necessary to walk on a wet or cluttered surface.

3.  I will not climb unless I am using a ladder or other appropriate equipment specifically designed for that purpose.

4.  I will not lift any load unless I know it can be done safely.

5.  I will immediately report all unsafe conditions to my supervisor.

6.  If required, I will wear personal protective equipment.

 

Follow Safe Work Practices at all times, however,  if an injury does occur -

 

SEEK MEDICAL TREATMENT FIRST!!!

Our First Responsibility Is to Take Care of the Injured Employee!

 

  Injured Employees are to go to the doctor immediately!

  DO NOT WAIT TO SEE IF YOU FEEL BETTER IN THE MORNING!

  GO NOW!!!!!!!!!!!!

  If the injured employee must make an appointment, it should be within the week or they should choose another doctor that is available to treat him/her as soon as possible. 

 

CALL-IN ACCIDENT REPORT

    When an injury occurs the campus designee (i.e. school secretary or  nurse)  not the injured employee should complete  the  ACCIDENT REPORT FORM that is required by the Safety Office.  It is important to take  time and to describe exactly what happened and why the accident occurred.

  Next, the accident form should be faxed to the Safety Office at 713-740-4046 before the employee reaches the doctor's office if at all possible.  This report is PISD's verification that the injury occurred at work. If the form is not completed at the time of the accident, then call the Safety Office at 713-740-0258 to give them at least the employee's name and type of injury.

  The doctor will call the Safety Office to get approval to see the patient, thus  the Safety Office cannot authorize the visit without the completed  Accident Report. 

    Information on the accident report helps the Safety Office in the timely filing of the First Report of Injury.

    

EMPLOYEES MAY CHOOSE THE DOCTOR

 •   If you are hurt at work and live in the Alliance service area, you are required to choose a treating doctor from the provider list.  This is required for you to receive coverage of healthcare costs for your work related injury.  A provider listing is available through the Alliance website at www.pswca.org and a link to that site is also contained on the Fund's website at www.tasbrmf.org.  It identifies providers who are taking new patients.

•   If your treating doctor leaves the Alliance, we will tell you in writing.  You will have the right to choose another treating doctor from the list of Alliance doctors.  If your doctor leaves the Alliance and you have a life threating or acute condition for which a disruption of care would be harmful to you, your doctor may request that you treat with him or her for an extra 90 days.

 

EMPLOYEE RIGHTS AND RESPONSIBILITIES UNDER THE TEXAS WORKERS' COMPENSATION SYSTEM AND ACKNOWLEDGEMENT FORM

  Each employee will receive a copy of the "Employee Rights and Responsibilities Under the Texas Workers' Compensation System" and the "PISD Safe Work Practices" at the time they report an injury.

  The injured employee should  sign the ACKNOWLEDGMENT FORM.

  Give the employee a copy of the signed acknowledgement form, keep a copy for your files and send the original to the Safety Office.

 

 BUILDING Accident Log

 Place a copy of ALL Call-In Accident Report Forms in your Building Accident Log even if the employee chooses not to seek medical treatment.

 

DAYS OF ABSENCE

  If an employee is absent because of an on-the-job injury, please let the Safety Office know immediately, as we must report it to the DWC.

  The first five days of an employee's absence from work due to an on-the-job injury shall be drawn from the employee's accrued time (sick leave, personal leave, and/or vacation).

•   Once all available leave is exhausted, an employee may be terminated.

 

PISD'S  RETURN TO WORK FORM

    As long as an injured employee is off duty, he/she must make an appointment and come to the Safety Office weekly.

  All injured employees must bring a doctor's release (work status report) to the Safety Office in the Administration Building before returning to work.

  Please have the employee ask the doctor's office to fax another copy of the work status report to 713/740-4046.

  A signed copy of the Return to Work Form will be given to the employee to give to his/her supervisor.
