Acknowledgement
|
The employee shall be given a copy of the "Employee Rights and
Responsibilities under the Texas Workers' Compensation System" which describes
the employees rights pertaining to benefits, doctors, confidentiality, and
responsibilities pertaining to their employer, doctors, insurance, and to
the Division. The employee can also contact the Texas Department of
Insurance, Workers' Compensation Division for more information or for assistance. The
original signed acknowledgement form should be forwarded to the Safety Office.
