ANNUAL VERIFICATION OF STUDENT INFORMATION ONLINE FORMS

The Texas Education Agency requires school districts to annually collect student information, health information, and emergency contact information. Parents/guardians of all new and returning students are required to provide this information by completing online registration using Skyward Family Access.

THE ANNUAL VERIFICATION ONLINE FORM NEEDS TO BE COMPLETED BEFORE SCHOOL BEGINS ON
AUGUST 13, 2024.

 


Parents of all new and returning students are required to complete online registration annually.

Student-specific online registration documents, which includes the name, username, and email address of the parents/guardians recorded in Skyward under Family 1, will be sent home the first day of school.

Contact your child's school if you need assistance  with any of the following?

  • Forgot Username and Password
  • Email address needs to be updated
  • All children in one family are not connected to the same account

Steps to Complete Online Registration for Returning Students for 2024-2025

  1. Log into Skyward Family Access

  2. Click on the green 2024-2025 Annual Verification icon.



  3.  Click on the Update Emer & Med Info icon to update your student's emergency and medical contact information.

IMPORTANT THINGS TO KNOW:

  • Items requiring an answer are marked with a red asterisk. (*)

  • You may stop the process and finish it at a later time.

  • You must click “submit” at the end of the process for your responses to be recorded and sent to the campus for approval.

  • Once the campus approves your documents, your responses will be reviewed to determine if your child qualifies for any additional programs and the emergency contacts you provided will be updated in Skyward.

  • IMPORTANT: Once the online form is submitted, do not re-open the form as it will undo the submission.
